If you are interested in being called upon to provide service, please fill out the online Master Service Agent  form and submit your information, or print it out  and either fax or mail it to our Warranty or Service Departments.  To be considered for the Master Service Agent Program you must agree to abide by the Cornelius and ICE O-Matic Warranty Policies and Procedures,

By submitting this information your company will be placed in a data base as a Service Agent (Sub-Contractor) and representative of GIS for service, installations and related issues for equipment own by a customer of GIS.  Many of the service issues are warranty related; however, as is often the case, your service contact with the end-user can lead to other services you provide that the customer can negotiate, such as preventative maintenance, etc...   Your company will also be listed as a prospect for invitations to service schools sponsored by a manufacturer.  We will also send service leads when the opportunity presents itself.
